Physical Therapy:

We accept Medicare, most PPO insurance and are in network with Anthem Blue Cross/Blue Shield.
When you call us we will obtain all of your insurance information and set up an appointment for your initial evaluation.
We then pre-authorize your treatment and call you within 24 hours to let you know your benefits and what your financial responsibility will be.

For your first appointment, please bring with you the following:

Prescription from your doctor for Physical Therapy

Please print and bring with you the following forms

Comfortable clothes to wear

Your co-pay if required
